Patient Information

The staff at Pairadocs is committed to providing excellent patient care. We strive to provide a relaxing and calm environment while you are at our office.

New Patient Experience

At your initial appointment, the clinician will gather information for establishing an individualized treatment care plan. This appointment will take on average two hours to complete. During this time, various needed X-rays, and intra and extra-oral photos will be taken. Your Periodontal exam will be conducted by your clinician, along with an oral cancer exam. Dr. Larry Bush will conduct a thorough comprehensive evaluation tailored to each patient’s needs and address any of your questions or concerns. A VelScope will be used as a preventative measure in detecting oral cancer. Dr. Bush will then recommend and prescribe a treatment plan as to what type of cleaning is best for your gingival tissue.

New Patient Forms

After scheduling a new patient visit, our Business Team will text a link to fill out paperwork.
This is a quick and easy way to add your information before your appointment time.
Please complete these before your first visit to our office.
If you have any questions regarding these forms, don't hesitate to get in touch with us at 810-230-8477.

Practice Policies

Financial Policy

Before treatment is performed, we will discuss your financial options so you will understand your financial responsibility. Payment is due when you schedule a future appointment and the time of service. If you have dental insurance, your benefits are determined by your employer, not your dentist. Your policy is a contract between you and your plan company. Your plan and payment are your responsibility. A plan is not a guarantee of payment; it often does not cover all the costs involved in treatment.

  • As a courtesy, we will contact your insurance provider to determine your coverage. We will also submit your claim and be in contact if we have questions. Please call our front desk at 810-230-8477 to see if we are working with your insurance provider.
  • Any deductible or estimated co-payment amount will be due at the time of treatment.

Cancellation Policy

We ask that you give our office 48-hours notice if you need to cancel or reschedule your appointment. Thank you for being courteous to our Business Team.

Payment Methods

We accept all major forms of payment including Care Credit financing.

We offer a 5% discount to patients who do not carry insurance and pay with cash or check.
